Job summary

Gentiva Hospice is seeking an experienced RN Case Manager to lead in-home end-of-life care. You will assess patients, develop and adjust plans of care, educate families, and coordinate with physicians and an interdisciplinary team to ensure compassionate, compliant care.

This role emphasizes clinical excellence, empathetic communication, timely visits, and thorough documentation to support patients and their families through a challenging time.

Qualifications

• RN with current license in the state of employment.
• CPR certification.
• Hospice or palliative care experience preferred.
• Minimum 1 year RN experience in acute care, oncology, long-term care, hospice or home health.

Responsibilities

• Perform comprehensive patient assessments and reassessments; educate patients, families and caregivers.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care and visit frequencies.
• Coordinate care across IDG; consult with physicians and other professionals.
• Educate patients and caregivers about disease, self-care, end-of-life care, medications and nutrition.
• Document timely clinical records and maintain compliance with infection control.
