Ideal candidates have 1+ year RN hospice/home health experience, CPR, and an active state license.

Qualifications

• RN with current license in the state of employment.
• Minimum 1 year of RN experience in hospice, home health, or acute care.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home settings.
• Develop and update plans of care with visit frequency and risk identification.
• Coordinate care with an interdisciplinary team for optimal outcomes.
• Provide education on disease, self‑care, and end‑of‑life care to patients and families.
