Job summary

Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead care for patients in home and community settings. You will assess, plan, implement, and document personalized end-of-life care, collaborating with physicians, social workers, chaplains, and other team members.

Responsibilities include assessments, symptom management, patient education, and caregiver support, with emphasis on compassion, accuracy, and regulatory compliance.

Qualifications

• Current RN license in the state of employment.
• One year of RN experience in hospice, home health, or related fields preferred.
• CPR certification required.
• Ability to travel to patients' homes and community settings.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across IDG and consult with physicians and staff.
• Educate patients and caregivers on disease, self-care, and end-of-life needs.
• Document clinical records timely and accurately; maintain compliance.
