Gentiva Hospice is seeking an experienced RN Case Manager to lead patient assessments and coordinate expert hospice care in patients' homes. You will document care plans, provide education to families, and collaborate with physicians, social workers, and other clinicians to coordinate services and ensure compassionate, compliant care.

The role emphasizes timely visits, accurate records, and support for end‑of‑life needs, including symptom management and bereavement guidance.

Formación

• Current license to practice nursing in the state of requested employment.
• CPR Certification.
• A minimum of one year experience as an RN (hospice/home health preferred).

Responsabilidades

• Be primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care based on each patient’s plan of care, in home or other settings.
• Perform assessments to monitor condition and adjust care as needed.
• Collaborate with an interdisciplinary team to build personalized care plans.
• Provide education, bereavement support, and caregiver training.
• Maintain clinical documentation and ensure infection control and compliance.
• Support quality improvement and scheduling initiatives and on-call activities.
