Job summary

Gentiva Hospice is seeking a dedicated RN to lead and manage patient care in home settings, ensuring compassionate hospice services. You will perform comprehensive patient assessments, implement care plans, and collaborate with physicians, LPNs, CNAs, social workers, chaplains, and other team members.

You will educate patients and families on disease progression, symptom management, and end-of-life care while maintaining high standards of documentation and compliance.

Qualifications

• Registered Nurse license in state of employment.
• CPR certification.
• Experience in hospice or palliative care preferred.

Responsibilities

• Assess and reassess patients in home or facility settings; provide education to patients and caregivers.
• Deliver and document skilled nursing care per Plan of Care.
• Coordinate with IDG to update care plans and schedules.
• Maintain accurate clinical documentation and ensure infection control compliance.
• Support on-call and patient support activities as applicable.
