Job summary

Gentiva in Alabama seeks an experienced Hospice RN / RN Case Manager to deliver and document skilled nursing care at patients' homes or wherever they call home. You will perform initial and ongoing assessments, coordinate with an interdisciplinary team, and provide education and bereavement support.

The role requires a current RN license and at least 1 year of RN experience, with hospice or palliative care exposure preferred. Competitive benefits and paid time off are included.

Qualifications

• Current license to practice professional nursing in Alabama.
• Minimum of 1 year RN experience in acute care, hospice, home health, or related field.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans and visit frequency with risk identification.
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients and caregivers on disease processes, medications, and end-of-life care.
• Communicate condition changes to the care team and document timely updates.
