Job summary

Gentiva is seeking a dedicated Hospice Registered Nurse Case Manager to join our full-time team in Alabama. You will lead nursing care in patients' homes, collaborating with an interdisciplinary team to craft personalized plans of care and support families through bereavement and education.

The role requires at least one year of RN experience, CPR, and an Alabama license. You will work Monday–Friday, with on-call as needed, serving Calhoun, Clay, Cleburne & Talladega counties with a focus on

Qualifications

Current license to practice nursing in the state of Alabama, or ability to obtain.
CPR Certification.
Current automobile insurance and valid driver’s license.
Hospice or palliative care experience preferred.
Minimum of one year RN experience in acute care, hospice, home health, or related settings.

Responsibilities

Provide and document skilled nursing care in patients' homes or appropriate settings according to the Plan of Care.
Perform initial and ongoing assessments; adjust care as needed.
Develop and update individualized plans of care with visit frequency to meet needs.
Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
Educate patients and caregivers on disease process, medications, and end-of-life planning.
Communicate changes in patient condition to the care team for timely interventions.
Maintain accurate clinical documentation and comply with infection control standards.
Participate in on-call and patient support activities as applicable.
