Job summary

Gentiva seeks a dedicated Hospice RN / RN Case Manager to provide compassionate nursing care in patients' homes or alternative settings. You will conduct assessments, implement the Plan of Care, and collaborate with a full interdisciplinary team to ensure high-quality end-of-life care.

You will educate families, manage symptoms, and document care accurately. The role requires an RN with at least 1 year of relevant experience, CPR certification, and a state nursing license.

Qualifications

• RN with a minimum of 1 year of RN experience in acute care, hospice, home health, or similar setting.
• Hospice or palliative care experience preferred.
• Current RN license in the state of employment; CPR certification.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per the Plan of Care.
• Develop and update individualized plans of care and visit frequencies.
• Coordinate care with IDG and physicians to implement care plans.
• Educate patients and caregivers on disease management, medications, and end-of-life care.
• Document clinical findings timely and maintain compliant records.
