Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to guide patients and families through the hospice journey. You will deliver skilled nursing care in patients' homes or other living settings, perform initial and ongoing assessments, and collaborate with an interdisciplinary team to create and refine personalized care plans.

You will educate patients and caregivers, document care plans, and ensure timely interventions while adhering to regulatory standards and quality initiatives.

Qualifications

• Current RN license in state of employment
• Hospice or palliative care experience preferred
• CPR certification required

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers on disease process, self-care, medications, nutrition and end-of-life care.
• Document timely clinical records and maintain compliance with infection control standards.
• Support quality improvement initiatives and care coordination across IDG.
• Participate in on-call and patient support activities as applicable.
