Job summary

Gentiva is seeking a Hospice RN/Case Manager to deliver compassionate, patient-centered nursing care in patients' homes, assisted living facilities, or where they reside. You will assess, plan, and implement personalized care plans, collaborating with the IDG to ensure quality, compliant end-of-life services.

Ideal candidates have RN licensure in Florida and at least 1 year of relevant experience, with hospice or palliative care preferred. Strong communication and documentation skills required.

Qualifications

• Registered Nurse credential required.
• 1 year RN experience in hospice, home health, or similar preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facility settings.
• Deliver skilled nursing interventions and symptom management per care plans.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care with an interdisciplinary team to optimize patient outcomes.
• Educate patients and caregivers on disease process, end-of-life care, medications, and nutrition.
