Gentiva seeks a compassionate Hospice RN / Case Manager to lead patient care at home or within supported living environments. You will assess needs, coordinate with the care team, and guide families through the hospice journey with dignity and clinical excellence.

Qualifications

• RN with current license in the state of employment.
• Minimum of one (1) year RN experience in acute care, hospice, home health, or related setting.
• Hospice or palliative care experience preferred

Responsabilités

• Be the primary point of contact for patients and families, guiding them through every step of the hospice journey.
• Deliver skilled nursing interventions and document care per Plan of Care in home, assisted living, or other home settings.
• Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
• Coordinate care across interdisciplinary teams to build and refine personalized care plans.
• Provide education, support, and bereavement guidance to patients and families.
• Teach caregivers how to care for their loved one safely and confidently.
• Communicate important updates in patient condition to the care team for timely interventions.
• Maintain accurate clinical documentation and follow infection control and regulatory standards.
• Support quality improvement and scheduling initiatives to ensure consistent, compassionate care.
• Assist patients and families in understanding hospice philosophy, symptom management, medications, and end-of-life processes.
