Job summary

Gentiva Hospice is seeking a Registered Nurse (RN) for Hospice care in Virginia Beach, VA. You will deliver skilled nursing care in patients' homes or care settings, collaborating with an interdisciplinary team to tailor plans of care and support families.

Requirements include Virginia RN license, CPR, and at least 1 year of RN experience in hospice, acute care, oncology, or home health; hospice/palliative care experience preferred.

Qualifications

• Active RN license in Virginia or ability to obtain.
• Hospice or palliative care experience preferred.
• CPR certification required.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including education for patients, families, and caregivers.
• Deliver skilled nursing care and symptom management.
• Develop and update individualized care plans with visit frequency.
• Coordinate care with interdisciplinary teams including physicians and other clinicians.
• Provide education to patients and caregivers on disease, self-care, medications, and end-of-life care.
