Qualifications

• RN license in the state of employment.
• CPR certification.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person or telephonic.
• Deliver nursing interventions and symptom management.
• Develop and update plans of care with visit frequency.
• Coordinate care across IDG and with physicians.
• Educate patients and caregivers on disease, end-of-life care, medications, and nutrition.
