Job summary

Ennoble Care is seeking a Hospice RN Case Manager to join our mobile care team serving Delaware County, PA. The RN will perform skilled nursing assessments, plan and provide care in collaboration with the interdisciplinary team to maximize comfort for patients and support families.

The role requires PA RN licensure, 1–2+ years nursing experience (preferably hospice), driving ability, and strong communication.

Qualifications

• Pennsylvania RN license required.
• 1-2+ years nursing experience; hospice preferred.
• Ability to assess needs of patients and caregiver families.
• Strong communication and documentation skills.
• Team-oriented with interdisciplinary collaboration.
• Valid driver’s license and reliable vehicle; ability to travel.
• Ability to work independently in a multidisciplinary setting.

Responsibilities

• Manage assigned patient caseload within territory; assess, plan, implement, evaluate patient care.
• Document notes in EMR; submit timely.
• Coordinate care with physicians, hospice staff, and other agencies.
• Educate caregivers on levels of care and patient needs.
• Collaborate with clinical leadership to address issues and improve care.
• Attend care conferences and participate in staff discussions.
