Compassus is seeking a Hospice Registered Nurse / RN Case Manager to join our compassionate care team in Leesport, PA. You will assess patient needs, coordinate interdisciplinary care, and support families to maintain comfort and dignity at home.

Requirements include an active RN license in the state, at least two years of nursing experience (hospice preferred), current driver’s license, CPR and BCLS certification, and a commitment to delivering high-quality, person-centered care.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse, hospice experience preferred.
• Current driver’s license.
• Current CPR and BCLS certification.
