Ennoble Care in New Jersey is seeking a full-time Registered Nurse Case Manager to support hospice patients in Cumberland County. You will work within the interdisciplinary team under the Director of Nursing to perform skilled nursing assessments, planning, and care to maximize comfort and health for patients and families.

The role requires NJ RN licensure, strong assessment and communication skills, and the ability to coordinate care with physicians, hospice staff, and community agencies.

Qualificações

• Graduate of an accredited School of Nursing.
• NJ RN license required by NJ State Board.
• One year of RN experience; hospice experience preferred.
• Strong assessment, problem-solving, and time-management skills.
• Valid driver's license with insured vehicle.
• Excellent written and verbal communication.

Responsabilidades

• Manage a defined patient caseload across the nursing process.
• Communicate with physicians, Hospice staff, and agencies to coordinate care.
• Document accurately in electronic medical records.
• Participate in Interdisciplinary Group meetings.
• Coordinate care during on-call time and bereavement periods.
