Job summary

Holisticare Hospice is seeking a dedicated Registered Nurse Case Manager to join our nursing team on a full-time basis in Delaware County communities. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of the hospice journey.

The role emphasizes compassionate care, clinical excellence, and collaboration with an interdisciplinary team. A strong nursing background, EMR proficiency, and a valid driver’s license are required.

Qualifications

• Active RN license
• Minimum 1 year RN experience in med-surg/acute care; hospice a plus
• Proficient in computers; EMR experience
• Current CPR certification if required by state
• Valid driver's license and reliable transportation

Responsibilities

• Provide hands-on hospice nursing care and coordinate services
• Collaborate with Clinical Director to assess client eligibility and care needs
• Educate patients and families to create personalized care plans
• Deliver compassionate, patient-centered nursing care and comfort measures
• Participate in call duties as needed to ensure 24/7 coverage
