Job summary

Ennoble Care is seeking a dedicated Hospice RN Case Manager to serve patients in Montgomery County, PA. The role involves managing a patient caseload, performing skilled nursing assessments, and coordinating interdisciplinary care to maximize comfort for patients and their families.

Qualifications

• Must hold a valid PA RN license.
• 1–2+ years in professional nursing, preferably hospice.
• Ability to assess needs of patients and families in varied settings.

Responsibilities

• Assess, plan, implement, and evaluate patient care across all nursing phases.
• Document notes in EMR accurately and timely.
• Coordinate with physicians, hospice staff, and other agencies for optimal care.
• Educate families on levels of care and care plans.
• Collaborate with clinical leadership to address issues and support growth.
• Participate in care conferences and case discussions as needed.
• Support patients and families during bereavement and at time of death.
• Engage in hospice orientation, training, and community programs.
