Requirements include PA RN licensure and 1–2+ years in hospice or home health. A valid driver’s license and ability to travel year‑round are essential for providing home visits and coordinating with physicians and caregivers.

Qualifications

• Active Pennsylvania RN license and clean driving record.
• 1–2+ years hospice or home health experience.
• Strong assessment, documentation, and communication skills.
• Ability to work independently and within a team.

Responsibilities

• Manage assigned patient caseload across territory, performing nursing assessments, planning, and care.
• Document all notes in EMR accurately and timely.
• Coordinate care with physicians, hospice staff, and other agencies.
• Educate families on levels of care for physical, emotional, and mental needs.
• Attend care conferences and participate in staff discussions.
• Supervise NAC every two weeks or as assigned.
• Provide support at time of death and bereavement.
• Participate in orientation and community programs.
