Job summary

Ennoble Care is seeking a Hospice RN Case Manager for North Philadelphia, PA. The RN Case Manager will perform skilled nursing assessments, care planning, and coordination with an interdisciplinary team to maximize comfort for patients and support for families.

The role requires PA RN licensure, 1–2+ years in nursing (hospice preferred), and a strong ability to document and communicate across departments. This is a full-time, on-site position with mileage reimbursement and benefits.

Qualifications

• Licensed RN in Pennsylvania.
• 1–2+ years in professional nursing; hospice preferred.
• Ability to assess patient and caregiver needs in varied settings.
• Strong documentation and reporting skills.

Responsibilities

• Manage assigned patient caseload within territory across all nursing phases.
• Document notes in EMR accurately and promptly.
• Coordinate with physicians, hospice staff, and other agencies for optimal care.
• Educate families on levels of care and best practices.
• Collaborate with clinical leadership to address issues and support growth.
• Attend care conferences and contribute to caseload discussions.
