Job summary
Holisticare Hospice is seeking a compassionate, full-time Hospice RN Case Manager (RNCM) to join our nursing team delivering exceptional end-of-life care across the community. You will provide hands-on nursing care, coordinate services, and guide patients and families through their hospice journey.
As part of our interdisciplinary team, you will work Monday–Friday 8 am–4:30 pm in the Delaware County service area, with a strong focus on clinical excellence, compassion, and clear communication,
Qualifications
- Current unencumbered RN license.
- Minimum 1 year RN experience in medical-surgical/acute care; hospice experience is a plus.
- Proficient with EMR and basic computer tasks; CPR certification if required by state.
- Valid driver’s license and reliable transportation.
Responsibilities
- Empower Through Engagement: respond to referrals with care and expertise.
- Collaborative Care Champion: assess eligibility for hospice services with Clinical Director.
- Nurturing Direct Care: deliver hands-on hospice nursing and coordinate care.
- Nursing with Heart: apply nursing skills to ease pain and provide comfort.
- Educator and Advocate: educate patients and families for personalized care plans.
- Call of Compassion: participate in call duties as part of the team.
