Job summary

UnitedHealth Group is seeking an RN Case Manager for Louisiana Hospice and Palliative Care, part of the LHC Group. You will deliver the Plan of Care for hospice patients, collaborating with the IDG and other professionals to promote physical, psychosocial, emotional, and spiritual well-being.

You will navigate diverse patient needs in a home setting. The role requires unrestricted RN licensure, valid driving credentials, and 2+ years of RN experience ( Hospice experience preferred).

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 2+ years of full-time RN experience required (hospice experience preferred).
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify patient/family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care; document goals, interventions and responses.
