Job summary

UnitedHealth Group seeks an experienced Registered Nurse Case Manager to provide hospice care in Louisiana. You will evaluate the Plan of Care, collaborate with a multidisciplinary team, and implement strategies to promote physical, psychosocial, emotional, and spiritual well-being for patients and families facing end-of-life care.

Responsibilities include initial evaluations, ongoing reassessment, care coordination, and patient/family education.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 2+ years of full-time RN experience.
• Two years of hospice experience may substitute for RN experience.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Re-assesses patient needs at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and responses.
• Collaborates with the patient, family, physician and IDG.
• Educates the patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes.
• Functions with diverse patient populations.
