Job summary

UnitedHealth Group seeks a Registered Nurse Case Manager to assume full nursing responsibility for the delivery of the Plan of Care for hospice patients. You will collaborate with the care team to implement treatment strategies that support physical, psychosocial, and spiritual well-being.

Qualifications include current RN licensure, CPR certification, 1+ year clinical experience, and a valid driver's license with access to reliable transportation.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current CPR Certification.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Ability to work with diverse patient populations and settings.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family responses.
• Collaborates with patient/family, attending physician and IDG members daily.
• Instructs and supervises patient/family in self-care when appropriate.
