Job summary

UnitedHealth Group's CHRISTUS VNA Hospice, part of the Optum family, is seeking an RN Case Manager to lead the Plan of Care for hospice patients and collaborate with the IDG and attending physicians. You will evaluate care needs, coordinate treatment strategies, and support families with compassionate nursing care.

Current RN licensure in applicable state, driver’s license, CPR certification, and 1+ year of clinical experience are preferred.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and re-assesses regularly.
• Initiates and coordinates the plan of care with the patient and family.
• Documents problems, goals, interventions, and responses to hospice care.
• Collaborates with patient/family, attending physician and IDG on daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes and adapts care to diverse patient needs.
