Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager for Louisiana Hospice and Palliative Care to oversee Plan of Care delivery for hospice patients. You will collaborate with the IDG and other professionals to ensure physical, psychosocial, emotional, and spiritual well-being.

Responsibilities include initial eligibility evaluation within 48 hours, care plan initiation, documentation, and patient/family instruction.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• 2+ years of full-time RN experience; two years of hospice experience as an LPN may substitute for RN hours.
• CPR certification or ability to complete within 90 days of hire (preferred).

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify and reassess patient/family needs at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and responses.
• Collaborate with patient/family, attending physician, and IDG daily.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes on patient condition.
• Provide care in diverse home situations regardless of demographics.
