Job summary

Louisiana Hospice and Palliative Care, part of LHC Group within the Optum family, invites you to join as a Registered Nurse Case Manager. You will deliver the Plan of Care for hospice patients, collaborate with the care team, and promote physical, psychosocial, emotional, and spiritual well‑being in end‑of‑life care.

Requires current RN licensure, a valid driver's license, and at least 2 years of full‑time RN experience (hospice experience can substitute).

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver's license and access to reliable vehicle or public transportation.
• 2+ years of full-time RN experience; hospice RN experience may substitute for RN experience.

Responsibilities

• Perform initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient/family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care with the IDG.
• Document problems, goals, interventions, and responses to care.
• Collaborate with patient/family, attending physician, and IDG members daily.
• Instruct patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes and adapt to home situations.
