Job summary

UnitedHealth Group is seeking a Hospice Registered Nurse Case Manager to deliver the Plan of Care for hospice patients in home settings. You will evaluate, coordinate, and implement treatment plans with the care team to promote physical, psychosocial, and spiritual well-being.

Requirements include an active RN license in the practice state, a valid driver's license and reliable vehicle (or access to transportation), and CPR certification or ability to obtain within 90 days.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Valid driver's license and access to a dependable vehicle (or public transportation).
• CPR certification or ability to complete within 90 days of hire.
• 1+ years of clinical experience (preferred).

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify the patient/family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with the patient/family, attending physician and IDG members daily.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes.
