Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize, and direct hospice care in home and community settings. Qualifications

• Graduate of National League of Nursing accredited school.
• Current RN licensure in the state.
• 1 year of professional nurse experience in last 3 years or a BSN with RN license; 2 years in public health/home health/hospice preferred.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Conduct initial and ongoing patient/family assessments to determine hospice needs.
• Provide professional nursing care using nursing processes.
• Initiate and revise plan of care as patient status changes; involve patient and family.
• Administer prescribed medications and treatments; deliver patient education.
• Coordinate care with physician, family, and interdisciplinary team; participate in meetings.
