Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize, and direct hospice care within patients’ homes and communities. The RN will assess needs, develop and revise care plans, administer medications, and educate patients and families in self-care and disease management.

The role requires collaboration with an interdisciplinary team, on-call participation, and ongoing professional development to maintain high-quality palliative care standards.

Qualifications

• Graduate of a National League of Nursing accredited school of nursing.
• Current licensure to practice as an RN in the state.
• RN with ≤3 years recent experience or BSN with RN license preferred.
• Supervisory experience not required.

Responsibilities

• Complete initial and ongoing comprehensive patient/family assessments.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient/family involvement.
• Administer medications and treatments per physician orders.
• Counsel patient and family; provide self-care education.
• Act as Case Manager when assigned; coordinate care.
• Communicate with physicians and hospice team; participate in meetings.
• Participate in on-call duties and ensure equipment/supplies availability.
