Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to guide patients and families through the hospice journey in home or community settings. You will deliver skilled nursing care, conduct assessments, and coordinate with physicians, social workers, and other professionals to implement personalized care plans.

Qualifications

• Current nursing license in the state of employment.
• CPR Certification.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits, with education for patients, families and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care and determine visit frequency.
• Coordinate care across interdisciplinary teams and communicate changes in condition to the team.
• Provide education on disease process, end-of-life care, medications, nutrition and self-care.
• Document timely clinical records and maintain infection control standards.
• Support transitions in levels of care and participate in on-call activities.
