Job summary

Gentiva seeks a compassionate Hospice RN / RN Case Manager to guide patients and families through the hospice journey in home and community settings. You will perform comprehensive assessments, develop plans of care, and deliver skilled nursing support tailored to each patient’s goals.

You will collaborate with physicians, LPNs, CNAs, social workers and chaplains, educate families, and ensure accurate documentation while upholding infection control and regulatory standards.

Qualifications

• RN license in the state of employment.
• Minimum 1 year of RN experience in hospice, home health, or related settings.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments; provide education to patients, families and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams with physicians and other clinicians.
• Educate patients and caregivers about disease, end‑of‑life care and medications.
• Communicate changes in condition to the care team and appropriate providers.
• Support transitions between home health, palliative and hospice levels of care.
• Document timely clinical records and maintain infection control/compliance standards.
• Participate in on‑call and patient support activities as applicable.
