Job summary

Gentiva Hospice is seeking a Registered Nurse to serve as a proactive RN Case Manager, guiding patients and families through the hospice journey in home, assisted living, or wherever they call home.

You will perform comprehensive assessments, deliver skilled nursing care, collaborate with an interdisciplinary team, educate caregivers, document care plans, and ensure compassionate, compliant care.

Qualifications

• Active RN license in the state of employment
• CPR certification (or as required)
• Hospice or palliative care experience preferred
• Minimum 1 year RN experience in relevant settings (hospice/home health)
• Must have transportation and valid driver's license

Responsibilities

• Perform comprehensive patient assessments and reassessments in person or via telehealth and educate patients, families and caregivers.
• Deliver skilled nursing interventions and symptom management per care plans.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams and consult with physicians.
• Provide education on disease process, self-care, end-of-life care, medications, nutrition, and dietary needs.
• Document clinical records timely and maintain infection control compliance.
• Support care continuity across home health, palliative and hospice transitions.
