Job summary

UnitedHealth Group, in partnership with Christus VNA Hospice (LHC Group), is hiring a Registered Nurse Case Manager. You will assume full nursing responsibility for the Plan of Care, evaluate performance, collaborate with professionals, and ensure compassionate, evidence-based care for hospice patients.

Requirements: current RN license in the state, valid drivers license with access to reliable transportation, and 1+ year of clinical experience.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current drivers license and access to dependable transportation.
• 1+ years of clinical experience.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient/family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response.
