Job summary

Optum is seeking a Registered Nurse Case Manager for hospice care within a respected post-acute partner network. You will evaluate, plan, and coordinate patient care under the Plan of Care, ensuring quality and productivity.

You will document progress and collaborate with families, physicians, and the care team to deliver compassionate, compliant care. Qualifications include an unrestricted RN license, 1+ year clinical experience, CPR certification, and a valid driver’s license with reliable

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current CPR Certification.
• Current driver's license and access to a dependable vehicle or public transportation.

Responsibilities

• Makes the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify physical, psychosocial, and environmental needs; re‑assess at least every 15 days.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document goals, interventions and patient/family response to hospice care.
• Collaborate with patient/family, physician and IDG members for daily care.
