Job summary

CHRISTUS VNA Hospice, part of the UnitedHealth Group family, is hiring a Registered Nurse Case Manager to deliver and manage the Plan of Care for hospice patients in the community. You will evaluate needs, coordinate care with the IDG, and implement treatment strategies to support physical, psychosocial, and spiritual well-being.

Qualifications include an active RN license in the state, a valid driver's license, and CPR certification. Prior hospice or home health experience is preferred.

Qualifications

• RN licensure in the state of practice (unrestricted).
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Preferred: 1+ years of clinical experience in hospice or home health setting.
• Current CPR certification or ability to obtain within 90 days.

Responsibilities

• Initial nursing evaluation within 48 hours of referral to determine hospice eligibility.
• Identify patient/family needs and reassess every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family response.
• Collaborate with patient/family and IDG members daily.
• Instruct and supervise patient/family in self-care techniques.
• Maintain accurate clinical notes.
• Function in any home situation regardless of demographics.
