Job summary

UnitedHealth Group, through Northern Arizona Hospice, part of the Optum family, seeks a Registered Nurse Case Manager to deliver the hospice Plan of Care and collaborate with the IDG to ensure quality, compassionate care for patients and families.

You will evaluate performance, implement strategies based on nursing theory, and promote physical, psychosocial, emotional, and spiritual well-being in patients and families. This is a full-time role with comprehensive benefits.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver license, valid vehicle insurance, and access to dependable transportation.
• CPR certification or ability to obtain within 90 days of hire (preferred).

Responsibilities

• Make the initial nursing evaluation within 48 hours of referral to determine hospice eligibility.
• Assess physical, psychosocial, and environmental needs and re-assess every 15 days.
• Initiate and coordinate the plan of care; document goals, interventions, and responses.
• Collaborate with patient/family, attending physician and IDG to provide daily care.
• Instruct and supervise patient/family in self-care techniques as appropriate.
• Maintain accurate clinical notes and adapt care across home settings.
