Job summary

Christus VNA Hospice is seeking an experienced Registered Nurse Case Manager to oversee the Plan of Care for hospice patients. You will evaluate performance, collaborate with other professionals, and implement treatment strategies that promote physical, psychosocial, emotional, and spiritual well-being in a home hospice setting.

Key duties include initial eligibility evaluation within 48 hours, ongoing needs assessment, and care coordination with the interdisciplinary group.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR Certification or ability to complete within 90 days of hire.
• Valid driver's license, and access to a dependable vehicle or public transportation.
• 1+ years of clinical experience.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services during visit within 48 hours of referral.
• Identifies patient/family needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care for hospice patients.
• Documents problems, goals, interventions, and patient/family response to care.
• Collaborates with patient/family, attending physician and IDG members in daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes regarding patient condition.
• Adapts to any home situation regardless of background to deliver care.
