Job summary

UnitedHealth Group is seeking a Registered Nurse Case Manager to lead the Plan of Care for hospice patients, collaborating with the care team to promote physical, psychosocial, and spiritual well‑being. You will evaluate performance and adjust care to improve quality and productivity.

The role requires RN licensure, CPR, and at least 1 year of clinical experience; a valid driver’s license and reliable transportation are preferred.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current CPR Certification.
• Driver’s license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• Ability to function in diverse home situations.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies patients’ needs and re-assesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and responses.
• Collaborates with patient/family, attending physician, and IDG daily.
• Instructs and supervises patient/family in self-care when appropriate.
• Maintains accurate clinical notes on patient condition.
