Job summary

Christus VNA Hospice, part of the Optum/LHC Group family, seeks a Registered Nurse Case Manager to deliver and manage the Plan of Care for hospice patients in a home setting. You will collaborate with professionals to implement evidence-based strategies that support physical, psychosocial, emotional, and spiritual well-being.

Experience with end-of-life care is valued. You will evaluate care, document outcomes, and coordinate with the patient, family, and IDG to meet patient goals while adapting

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current driver''s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• 1+ years of clinical experience.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identifies the patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care.
• Documents problems, goals, interventions, and patient/family response to hospice care.
• Collaborates with patient/family, attending physician and IDG members to provide daily care.
• Instructs and supervises patient/family in self-care techniques when appropriate.
• Maintains accurate clinical notes on patient condition.
• Works with the care team to ensure care aligns with the Plan of Care and patient goals.
