Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to lead and coordinate care for patients at home or in care facilities. You will assess needs, implement Plan of Care, and collaborate with an interdisciplinary team to ensure compassionate end-of-life care.

The role emphasizes education for patients and families, accurate documentation, and adherence to regulatory standards while supporting quality improvement initiatives.

Qualifications

• Valid RN license in the state of employment.
• Minimum 1 year RN experience in acute care, oncology, long-term care, hospice, or home health.
• Hospice or palliative care experience preferred.
• CPR certification required.
• Current automobile insurance and valid driver’s license.
• Knowledge of hospice care and interdisciplinary team coordination.
• Excellent patient assessment and communication skills.

Responsibilities

• Be primary contact for patients and families, guiding their hospice journey.
• Deliver and document skilled nursing care per Plan of Care in home or facility.
• Perform initial and ongoing assessments; adjust care as needed.
• Coordinate care with IDG including physicians, LPNs, CNAs, social workers.
• Provide emotional, physical, and spiritual support to patients and loved ones.
• Educate caregivers on safe, confident home care.
• Communicate significant changes in patient condition to the care team.
• Maintain timely clinical documentation and infection control standards.
• Support quality improvement, scheduling, and transitions of care.
• Assist patients and families with hospice philosophy, symptom management, and end-of-life processes.
