Qualifications

• RN with a minimum of 1 year experience in hospice, home health, or related setting.
• Experience in patient education and care planning.
• Strong communication and teamwork abilities.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans including visit frequency.
• Coordinate care with physicians and the IDT.
• Document timely clinical records and ensure regulatory compliance.
