Job summary

Gentiva is seeking an experienced Hospice RN/Case Manager to lead personalized end-of-life care in patients' homes or care facilities. You will assess, plan, implement, and document skilled nursing care with compassion and clinical excellence, coordinating with physicians, CNAs, social workers, and chaplains.

You will educate families, support them emotionally, and ensure compliant documentation and scheduling with a patient-centered approach.

Qualifications

• RN license in state of employment.
• Hospice/palliative care experience preferred.
• 1+ year RN experience in acute care, hospice or home health preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments; educate patients, families, and caregivers.
• Deliver nursing interventions and symptom management per Plan of Care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with interdisciplinary teams and physicians.
• Educate patients and caregivers on disease, care and end-of-life needs.
• Document all care activities and maintain up-to-date records.
• Participate in on-call and remote support as applicable.
