Job summary

Heartland Hospice, part of Gentiva, seeks an experienced Hospice RN / RN Case Manager to lead in-home end-of-life care. You will assess patients, develop plans with an interdisciplinary team, educate families, and provide compassionate support in home settings.

Responsibilities include coordinating care, updating plans, and maintaining precise documentation while upholding infection control standards and regulatory requirements.

Qualifications

• RN license in good standing.
• CPR Certification.
• Experience in hospice or palliative care preferred.

Responsibilities

• Be the primary point of contact for patients and families, guiding them through every step of the hospice journey.
• Deliver and document skilled nursing care based on each patient’s Plan of Care, in home or other settings.
• Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
• Collaborate with interdisciplinary team to build and refine personalized care plans.
• Provide emotional, physical, and spiritual support to patients and loved ones; educate caregivers for home care.
• Maintain accurate clinical documentation and ensure infection control compliance.
• Support quality improvement and scheduling initiatives for consistent, compassionate care.
• Explain medications and end-of-life care concepts to patients and families.
