Heartland Hospice is seeking an experienced RN Case Manager to lead in-home hospice care. You will conduct comprehensive RN assessments, implement and document care plans, and collaborate with an interdisciplinary team to support patients and families.

The role emphasizes education, symptom management, and seamless communication with physicians, nurses, and social workers to ensure compassionate, high-quality care in patients' homes.

Qualifications

• Current RN license to practice in the state of employment.
• Hospice or home health experience preferred.
• Minimum 1 year RN experience in hospice, palliative, home health, or acute care beneficial.

Responsibilities

• Provide skilled nursing care in patients' homes per Plan of Care.
• Perform initial and ongoing RN assessments and adjust care as needed.
• Coordinate care with IDG, physicians, and support staff.
• Educate patients and caregivers on disease, treatment, and self-care.
