Job summary

Heartland Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead patient care in home and community settings. You will assess, plan, and coordinate end-of-life care with an interdisciplinary team, delivering compassionate nursing interventions and education to patients and families.

The role requires an active RN license and at least 1 year of hospice/home health experience, with CPR certification and a valid driver’s license.

Qualifications

• RN license in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or similar settings.
• CPR certification and valid driver’s license.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver skilled nursing interventions and symptom management.
• Develop and update plans of care with visit frequency.
• Coordinate care across interdisciplinary teams.
• Educate patients and caregivers on disease, end-of-life care, medications, and nutrition.
