Gentiva Hospice seeks a compassionate Registered Nurse to serve as a Hospice RN / RN Case Manager, delivering skilled nursing care in patients’ homes and to support families through end-of-life care. You will assess, plan, and adjust care with the IDG to ensure comfort and dignity.

Responsibilities include education for families, timely documentation, and collaboration with physicians, social workers, and chaplains to optimize patient outcomes and satisfaction.

Qualifications

• RN license in the state of employment
• Hospice or palliative care experience preferred
• Strong assessment and documentation skills

Responsabilités

• Perform comprehensive patient assessments and reassessments in home or facility settings
• Deliver and document skilled nursing care per individualized plans of care
• Coordinate with IDG to refine care plans and ensure timely interventions
• Educate patients, families, and caregivers on disease, end-of-life care, and medications
