Job summary

Heartland Hospice, part of Gentiva, seeks a Nursing professional to serve as a Hospice RN / RN Case Manager. You will provide compassionate, skilled nursing care in patients’ homes and coordinate with physicians, social workers, and other team members to uphold individualized care plans.

Ideal candidates hold an active NC RN license with hospice or home health experience and excel in patient education, empathy, and clinical documentation. Sign‑on bonus available.

Qualifications

• Active RN license in North Carolina.
• Hospice or palliative care experience preferred.
• Strong communication and caregiving skills.

Responsibilities

• Be the primary contact for patients and families, guiding them through hospice.
• Deliver and document skilled nursing care in patients' homes or facilities.
• Perform initial and ongoing assessments to monitor patient condition.
• Collaborate with an interdisciplinary team to plan and adjust care.
• Provide education and bereavement guidance to families.
• Teach caregivers practical skills for safe, confident care.
• Maintain timely clinical documentation and infection control compliance.
• Support quality improvement and scheduling initiatives.
