Gentiva’s Heartland Hospice seeks a dedicated RN Case Manager to lead in-home hospice care, guiding patients and families through the care journey with compassion.

The role includes performing assessments, delivering skilled nursing, coordinating with physicians, social workers, and other team members, and educating families on end-of-life care while ensuring compliant documentation and safety.

Qualifications

• Active RN license in the state of employment.
• 1+ year RN experience in hospice, palliative, home health, or related setting preferred.
• Ability to educate patients, families, and caregivers effectively.

Responsibilities

• Perform comprehensive in-home patient assessments and ongoing reassessments.
• Deliver skilled nursing interventions and symptom management per the Plan of Care.
• Coordinate care with IDG to develop and update individualized plans of care.
