Gentiva seeks an experienced Hospice RN / RN Case Manager to provide compassionate, patient-centered care at home in Oshkosh, WI. You will assess, plan, implement, and document care with an interdisciplinary team to ensure comfort and family support.

A $5,000 sign-on bonus is offered, with opportunities for professional growth and advancement. The role emphasizes timely assessments, education for families, and coordination of services in home settings. Wisconsin RN license is required.

Qualifications

• Licensed RN in Wisconsin required.
• Hospice or home health experience preferred.
• One year RN experience in hospice/acute care settings preferred.

Responsibilities

• Perform comprehensive patient assessments and ongoing assessments in home settings.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans with visit frequency.
• Coordinate care with interdisciplinary team and physicians.
• Educate patients and families on disease process, end-of-life care, medications, nutrition.
• Communicate changes in condition to care team.
• Support continuity of care across transitions in levels of care.
• Document clinical records accurately and timely.
• Maintain infection control and regulatory compliance.
• Participate in on-call and patient support activities as applicable.
